The Psoas Part 3: Management
In part I of The Psoas Syndrome, we discussed the 3 myths of the psoas. In part II we looked at the functional role of psoas.
In part III, we will look at a quick hip assessment and a simple yet incredibly effective conditioning pattern for the psoas.
As we mentioned in part II, the psoas functions mainly as a spinal, pelvis, and hip stabilizer.
Because of its location and function in both spine stabilization and hip centration, the psoas is a challenging muscle to isolate and condition. Since the psoas fascially blends with both the diaphragm and pelvic floor we will include both breathing and core activation in its’ conditioning.
Prior to conditioning, the client must be assessed for their ability to dissociate their hip from their pelvis. The inability to dissociate the hip from the pelvis will lead to dysfunction during any lower extremity pattern and perpetuate psoas dysfunction.
We will use the bent knee fall out to assess the client’s ability to dissociate their hip from the pelvis. If they are unable to effectively dissociate their hip, the client may require soft tissue release of the hip musculature or joint capsule. If upon evaluation they have full hip joint range of motion, they require improved core stability and the next pattern will be extremely useful at providing this stability.

CONCLUSION
The psoas is an extremely unique muscle functioning in a variety of roles:
- as a connection between the trunk and the lower extremity
- as a stabilizer of the spine, pelvis, and hips during activities of daily living
- as a decelerator of the trunk and hip during gait.
Understanding these roles can help you improve the patterns of your client with chronic dysfunction, improve your client that is already performing at a high level, and establish yourself as well paid and sought-after specialist.
